If lines around your mouth have started to leave a lasting impression—literally—you’re not alone. These lines can make you look tired, stressed, or older than you feel. At Plastic Surgeons of Alaska, we often see patients looking for ways to smooth out these stubborn creases, and BOTOX is one of the tools we may recommend, depending on your needs.
What Are Frown Lines and Marionette Lines?
Let’s start by clarifying what we mean by frown lines and how they differ from marionette lines.
- Frown lines usually refer to the vertical “11” lines between your eyebrows, but they can also describe creases that form around the mouth when the corners are pulled downward by facial muscles.
- Marionette lines are the lines that run from the corners of your mouth toward your chin. Over time, these lines can become more pronounced, giving the lower face a drooping or sad expression.
Both types of lines are caused by repeated facial movement, gravity, and the natural loss of skin elasticity and volume. The key difference lies in their location and the factors responsible for forming them.
How BOTOX Works on Lines Around the Mouth
BOTOX is a neuromodulator that temporarily relaxes targeted facial muscles. It softens the appearance of lines caused by repeated facial expressions—like frowning, grimacing, or even smiling. In the lower face, a common target is the depressor anguli oris (DAO) muscle, which pulls the corners of the mouth downward.
When BOTOX is carefully injected into the DAO muscle, it can relax the downward pull and subtly lift the corners of the mouth. This helps you look more relaxed and less tense without creating an exaggerated or frozen appearance.
Where BOTOX Is Injected & How Much You Might Need
Everyone’s face differs, so the number of units needed and the exact injection sites can vary. However, here are some general guidelines:
Downturned Corners of the Mouth
- Injection site: DAO muscle at the outer corners of the mouth
- Units typically needed: 2 to 4 units per side
- Treatment time: Around 10 to 15 minutes
- Results: Often noticeable within a few days, lasting 3 to 4 months
Marionette Lines
- BOTOX may help when muscle activity is causing the lines. However, if volume loss is the main issue, your provider may recommend dermal fillers instead—or a combination of both. The cost depends on the amount of BOTOX or fillers used. During your consultation, your provider will discuss a custom plan and pricing so you know what to expect.

Combining Treatments for the Best Results
While BOTOX is excellent for treating dynamic wrinkles caused by muscle movement, it’s often not the entire solution for deep lines on each side of your mouth. Many benefit from combining BOTOX with dermal fillers to comprehensively restore a smoother, more youthful look. Together, these treatments can enhance your results while maintaining a natural, refreshed appearance.
